Federal Funding Opportunity Cover Sheet

Use the cover sheet form for all pre-proposal and full-proposal submissions.  To digitally enter text and save your information, you must download the form in a full version of Adobe Acrobat. If you lack the full version, you will not be able to save your changes to this cover sheet form. (Adobe Acrobat Reader will not allow you to save your changes.)

A completed cover sheet is required for every pre-proposal and full-proposal submission.

Cover Sheet Instructions

The OER Proposal Cover Sheet has been designed to categorize information about pre-proposals and full- proposals for entry into a database. Having this information in a database allows the office to better organize and manage information about proposed explorations.

The following instructions are provided to assist you with completing the form, and to ensure that the information you enter best summarizes and describes your pre-proposal and/or full-proposal.

Title - Provide a short descriptive title for the proposed project.

Thematic Categories – Choose the appropriate Thematic Priority that best represents your proposed exploration.

Contact Information

1. Contact Information - Provide name and contact information for the Principal Investigator (PI) and primary Co-PIs. These will be our primary contacts as the proposal is processed.

2. Significant Participants - List up to 2 significant participants and identify their institutions. These are people who will play a significant role in planning and executing the work. There is no need to identify the entire science party for an expedition on the cover sheet.

Budget Summary

1. # Years – Indicate if your proposal is for 1 or 2 years.

2. FY14 OER Request - Provide the total ($) amount requested from OER to support the proposal in FY 2014 (1 October 2013 to 30 September 2014).

3. Total OER Request - Provide the total ($) amount requested from OER to support the proposal over multiple years if it is a multiyear proposal. Multiyear length of award is limited to two years.

Cost Share - Note that cost share or matching funds are not required, but are encouraged.

A. Collaborations -

  1. Identify collaborating institutions that are contributing to the proposed effort and the total ($) amount of the contribution.

  2. Categorize the nature of each contribution using the following:

    • Ship time

    • Submersible time

    • Salaries

    • Equipment/supplies

    • Data processing/lab work

    • Complementary grant

    • Travel

    • Product development

    • Other - describe

If a collaborating institution is contributing in more than one way (e.g., providing ($) for salaries and data processing/lab work), please provide separate entries. Note that a detailed accounting of the collaboration is not required as part of the cover sheet.

Field Operations Information

1. Field Dates - Please provide known or estimated desired start and end dates for field operations.

2. # Total Field Days - Please provide the total number of operating days required to conduct the work.

3. Max. Depth - Provide the approximate maximum water depth for field operations.

4. Approximate Lat/Long - Provide approximate latitude and longitude of the center of the area in which field operations will take place.

5. Geographic Region - Select the major basin where the field operation will occur, and identify a place name if available (e.g., select Western North Atlantic then write in Charleston Bump). Do not identify geographic regions for anything other than a field operation.

Vessel Information

Option 1 - If you have access to a vessel and do not require OER funding:

  1. Identify the vessel

  2. Identify the institution that operates the vessel

  3. Identify the cost ($) per day

  4. Identify the total ($) cost to cover the vessel

  5. Ensure the ($) amount is included in collaborations (see Cost Sharing above),
    Ensure that the ($) amount is not included in the FY14 OER funding request (see section 2 item 1 above).

Option 2 - If you have access to a vessel and you require OER funding:

  1. Identify the vessel

  2. Identify the institution that operates the vessel

  3. Identify the cost ($) per day

  4. Identify the total ($) cost to cover the vessel

  5. Ensure the ($) amount is included in your total ($) request.

Submersible Information – Use the list to identify any submersibles that you will be using during field operations.

Project Activities - Use the list to identify the types of activities that you will be conducting during field operations.

Systems - Use the list to identify the primary systems that you will be using to gather data and information during field operations.

Education and OutreachOER expeditions engage in a variety of education and outreach activities. Please indicate which activities you would like to participate in or contribute to prior to, during, or after proposed field work.

Products and Outcomes - Use the list to identify any products that you anticipate developing as a result of your work.

Additional Comments: - Only use this section to provide additional information to answer questions on the cover sheet. Do not include any item or effort that is already part of the proposal.

Instructions for Submission of the Cover Sheet - The cover sheet is required for pre-proposal and full-proposal submissions. First, complete the cover sheet by following the directions on the form. When you press “FINISH” choose the Internet email option and save the document as an .xml file on your computer. Note, this action will not submit the cover sheet by email. It only provides you the option to save the cover sheet as an .xml file.

1) For pre-proposal submission, attach the .xml file along with the completed pdf cover sheet and your pre-proposal document to an email and send to: oer.ffo2014@noaa.gov.

2) For full-proposal submission, upload the .xml file and the completed pdf cover sheet to Grants.Gov as part of the application process.
